You could never accuse the Soviets of having good quality control with their weapons or anything else for that matter. Their whole military-industrial complex was geared toward quantity not quality.

No but at the minimum you had central command and control. With the break up, they never accounted for all the weapons, never accounted for all the material. I read a book a while back written by a Soviet Nuclear Scientist defector which outlined a lot of scary problems facing the Russians.
