Odd question...anybody have a book of church tunes

I'm looking for the chords and all that bunk for the church tune "Hear Us Now God And Father". Is anybody able to supply me with it? I know we have some of you church goin' folks around here.

 

(anybody ever notice that if you say it fast enough it sounds like "Hear us now goddamn father"? My sister-in-law told me the title over the cell phone and I sat there stunned for a few seconds about that being the name for a song at her wedding.)
