I like the MLB.tv option, blackout rules notwithstanding. It's light-years ahead of the NFL.

The thing is, it's not hard at all to "geolocate" people by IP address and stream ONLY their local coverage with local ads and all... as if you really were watching it on TV. Maybe then they have a payed version level for out of market games.
