Depends on the situation and team from the ones I worked with. Typically the pitching coach will know more about the mechanics and fatigue signs so he is usually the first one. But sometimes the Manager was a pitching coach so it may be him. If it's just performance than the manager may go first. But most often it's the pitching coach suggestions. Guys like TLR whose background is not pitching tend to let the pitching coach do his thing.

I think towards the end they all seemed gassed. The knee injury was not on Jatz unless you want to stretch it to say he may have injured it due to fatigue, I wouldn't. I would put kopech in that group. They were trying to ramp up his innings but probably started to early. Overall it's wasn't that they were all injured but more that their performances all tailed off toward the end of the season. They even tried to decrease the innings and skip starts at the end of the season but it was too late by then. Overall I'm all for increased work for starters but last year and this year are exceptions where the preseason preparation isn't there and alteration to the typical program need to occur.

The pitching coach determines the innings, schedule, between game throwing and basically everything to do with pitching. They really bring the medical staff in when issues arise.

Offseason Part 3 - Because Part 2 Was a Dud
I just going on his comments about treating last year like any other year. The "pre season" was so messed up and COVID caused so many issues, he should not have pushed them physically so early. All of the performance improvements won't matter if they can't pitch. The greatest ability is availability. As I said hopefully he learned. That was his first time working as the primary pitching coach. He now needs to make sure they are ready for a full season and with the disadvantage of a shortened Spring Training.

Over the last 15-20 years there haven't been very many shortened spring trainings when the pitchers aren't ready to go. Physiology is Physiology. There are some things that don't change. Pitchers so need to throw more in general but not when they aren't ready.

They need to make sure the best pitchers aren't overworked early in the year. Not sure if it's acquiring an SP or just many calls to the pen. The game thread are going to freak when pitchers are pulled in the 4th and looking good but it's going to need to be done.

You have wonder if this came from earlier kimbrel trade discussions that fell through but the Sox liked this guy as part of that deal and found another way to get him.
