According to this post it's only happened 6 times in MLB history.

They've hit 4 in an inning four separate times. The last time being 8/27 last year against Tampa Bay.

His defense allowed those two runs.

 

But, he threw a lot of pitches to every batter and, he could use a little work on his location.
