That lack of institutional control is apparently a bad phrase because it shows a repeat offense and places blame on the university's compliance dept. The punishment does seem harsh though. -

I think we pulled down about 3-5 episodes a night a times.

Just saw on SC that Boise has some major violations reporting the dreaded "lack of institutional control". They said it involves more than just the football team though. -
